On the lowest level the keyboard electronic sends scan codes for *every* single action like "key pressed" or "key released". It doesn ot distinguish between Ctrl/Alt/Shift or other keys. All are handled in the same way. The keyboard driver on the pc then uses these codes to build a higher level interface for programs which then see a Ctrl+F4 as only one key (although it is still possible to retrieve whether the Ctrl/Alt/Shift etc. key is currently pressed or not). The keyboard itself not even switches any numlock/shiftlock/capslock light on. This is all done by the keyboard driver.
